  "abstract": "Background This study aimed to identify the independent risk factors for moderate-to-severe acute gastrointestinal injury (AGI) in septic patients and to construct and validate a practical predictive model for early risk stratification.Methods A retrospective cohort study was performed using data from the MIMIC-IV database. A total of 1,200 septic patients were included as the development cohort. Multivariable logistic regression was employed to identify independent predictors, which were integrated into a nomogram. Its performance was internally validated via bootstrapping and externally validated using an independent cohort of 253 patients. A Random Forest model was also developed for comparison.Results Seven factors were independently associated with moderate-to-severe AGI: mechanical ventilation, multiple organ dysfunction syndrome (MODS), higher admission SOFA score, higher admission APACHE II score, prolonged vasopressor use, elevated C-reactive protein (CRP), and elevated intra-abdominal pressure (IAP). The nomogram demonstrated excellent discrimination, with a C-index of 0.862 (internal validation) and 0.858 (external validation), and good calibration. It achieved an area under the curve of 0.829, with a sensitivity of 86.64% and a specificity of 88.17%. Compared to the Random Forest model, the nomogram showed superior sensitivity and discriminatory ability.Conclusions Mechanical ventilation, MODS, high illness severity scores, prolonged vasopressor use, elevated CRP, and high IAP are significant predictors of moderate-to-severe AGI in ­sepsis. The developed nomogram provides a reliable and individualized tool for early risk prediction, potentially aiding timely clinical intervention to improve patient outcomes.",
