  "abstract": "Background To characterize research trends and emerging hotspots in organ preservation for rectal cancer after neoadjuvant therapy using bibliometric analysis.Methods Articles on organ preservation in rectal cancer after neoadjuvant therapy were retrieved from the Web of Science Core Collection from inception to June 23, 2023. After screening, 573 publications were included (IDDF2026-ABS-0357 ­Figure 1). Bibliometric indicators, including annual output, publication type, countries, institutions, authors, and keywords, were analyzed and visualized using CiteSpace. Annual publication trends and document types are shown in Figure 2A-B ( IDDF2026-ABS-0357 Figure 2(A,B)); collaboration ­networks of countries, institutions, and authors are presented in Figure 3A-C (IDDF2026-ABS-0357 Figure 3(A-C)). Keyword co-­occurrence, clustering, timeline evolution, and citation bursts were used to identify research hotspots and frontier topics (IDDF2026-ABS-0357 Figures 4-6).Results A total of 573 publications were analyzed. Annual output generally increased from 2014 to 2023, with articles accounting for 78% and reviews for 22% ( IDDF2026-ABS-0357 Figure 2(A,B)). The United States produced the largest number of publications (n = 117), followed by China (n = 87). Maastricht University was the most productive institution (n = 49) (IDDF2026-ABS-0357 Figure 3(A,B)). Author collaboration analysis identified several active research groups in this field (IDDF2026-ABS-0357 Figure 3(C)). Keyword analyses showed that the research focus has shifted from conventional treatment strategies, including surgery and chemoradiotherapy, toward organ-preservation approaches centered on watch-and-wait, complete clinical response, imaging assessment, and tailored neoadjuvant treatment (IDDF2026-ABS-0357 Figure 4(A,B); IDDF2026-ABS-0357 Figure 5(A,B)). Citation burst analysis further suggested that ‘GRECCAR 2,’ ‘international watch,’ ‘clinical complete responders,’ ‘WAIT database,’ and ‘long-term outcome’ have emerged as recent frontier topics (IDDF2026-ABS-0357 Figure 6). In addition, immunotherapy and health economics appear to be developing themes with potential future relevance.Conclusions Research on organ preservation in rectal cancer after neoadjuvant therapy has expanded steadily over the past decade. Current hotspots focus on watch-and-wait strategies, response evaluation, individualized neoadjuvant treatment, and long-term outcomes.
