  "abstract": "Background This study aimed to evaluate the predictive value of the Systemic Immune-Inflammation Index (SII) for the early detection of Acute Gastrointestinal Injury (AGI) in patients with sepsis. By investigating the relationship between SII and the severity of AGI and intestinal mucosal barrier function, this study provides a basis for early diagnosis and intervention to improve patient outcomes.Methods This retrospective study included 1,273 sepsis patients from the MIMIC-IV database. Patients were divided into an AGI group and a non-AGI group. Data including blood urea nitrogen, creatinine, blood glucose, neutrophils, lymphocytes, the Systemic Inflammation Response Index (SIRI), and SII were compared. Logistic regression and Receiver Operating Characteristic (ROC) curve analyses were used to assess the predictive value of SII for AGI onset. Spearman’s correlation analysis was employed to determine its association with AGI severity.Results Among the 1,273 patients, 683 (53.65%) developed AGI. The SII was significantly higher in the AGI group compared to the non-AGI group (P < 0.05). Spearman’s correlation analysis showed a moderate positive correlation between SII (r=0.5428, P<0.05) and SIRI (r=0.4911, P<0.05) and the occurrence of AGI. Multivariate logistic regression analysis identified SII (β=0.6571, OR=1.84, P<0.001) as an independent predictor for AGI, along with SIRI. ROC analysis demonstrated that SII had strong predictive ability for AGI occurrence (AUC=0.83, sensitivity=0.82, specificity=0.75).Conclusions The SII is a reliable biomarker for the early detection of AGI in sepsis patients, demonstrating robust predictive performance. These findings suggest that SII may facilitate early risk stratification and clinical decision-making, potentially improving patient prognosis.",
