  "abstract": "Background Approximately 5–10% of all patients with metastatic colorectal cancer (mCRC) harbor a BRAF V600E mutation. These patients exhibit distinct metastatic patterns, poor prognosis, and heterogeneous survival outcomes. The findings from the BREAKWATER study indicated that administering chemotherapy plus encorafenib and cetuximab as first-line treatment extended the median duration of progression-free survival (PFS) and overall survival (OS). The purpose of this analysis was to report the efficacy and safety data of administration of chemotherapy plus dabrafenib and cetuximab as first-line and second-line treatment in BRAFV600E-Mutant mCRC from a single center.Methods We retrospectively reviewed the medical records of 22 patients who had received a diagnosis of BRAF V600E-mutant mCRC between November 2023 and November 2025. All patients underwent BRAF V600E genotyping before chemotherapy plus Dabrafenib and cetuximab. The primary endpoint was objective response rate (ORR), and secondary endpoints were PFS and adverse events (AEs). The final analysis of the database was last updated on February 2 nd, 2026.Results In the first-line group, the ORR was 83.33%; while in the second-line group, the ORR was 20.00%. The DCR was 100% in both groups. In the pooled analysis, the most common grade 1-2 AE were anemia (72.73%), fever (27.27%), neutropenia (31.82%), leukopenia (45.45%), elevated aspartate aminotransferase level (27.27%), hypoalbuminemia (36.36%), hypomagnesemia (36.36%), and hypokalemia (36.36%). In the pooled analysis, the most common grade 3-5 AE were neutropenia (18.18%), intestinal obstruction (13.64%), and elevated alanine aminotransferase level (13.64%). In the first-line group, half of the patients were progression-free at the final follow-up. In the second-line group, 8 out of 10 patients were progression-free at the final follow-up.Conclusions Our findings indicate that Dabrafenib and cetuximab with chemotherapy are effective and tolerable first-line and second-line regimens for BRAF V600E-Mutant metastatic colorectal cancer patients. Furthermore, earlier use and combination with chemotherapy would achieve better response.",
