  "abstract": "Background Pancreatic exocrine insufficiency (PEI) has been linked to an elevated risk of fatty liver disease. However, its relationship with metabolic dysfunction-associated fatty liver disease (MAFLD) in the general population is not well-defined. This study aims to characterize and compare the clinical profiles of patients with different combinations of PEI and MAFLD status.Methods The study cohort was divided into three groups according to the PEI and MAFLD status: Group ( FL/PEI), patients with both MAFLD and PEI; Group (FL), patients with only MAFLD; Group (PEI), patients with only PEI. Flowchart of the research process)Results Overall, 669 cases (mean age 69.37 ± 6.34 years, 41.11% men) were recruited. Among PEI patients, obese individuals face a significantly higher risk of MAFLD compared to those with a normal body mass index (BMI; OR 4.061, 95%CI 1.813 to 9.097, P = 0.001). In patients with MAFLD, males had a significantly higher risk of PEI (OR 1.856, 95%CI 1.061 to 3.247, P = 0.030).Conclusions Impaired cholesterol absorption may be a cause of PEI-induced fatty liver.
