  "abstract": "Background Non-alcoholic fatty liver disease (NAFLD) is increasingly prevalent and strongly associated with obesity and metabolic syndrome. Lifestyle modification remains the cornerstone of management, yet real-world evidence on its effectiveness in primary care is limited. This study aimed to evaluate the impact of a structured diet and exercise program on early-stage NAFLD.Methods A prospective six-month study was conducted in 120 adult patients with ultrasound-confirmed mild NAFLD. Participants received individualized dietary counseling focused on calorie reduction and balanced macronutrients, combined with supervised physical activity three times per week. Baseline assessments included liver function tests (LFTs), body mass index (BMI), and hepatic fat grading by ultrasound. Follow-up assessments were performed at the end of the intervention period. Adherence to diet and exercise recommendations was recorded.Results After six months, 65% of participants showed normalization or improvement in liver enzyme levels. Ultrasound revealed a reduction in hepatic fat in 48% of patients. Mean BMI decreased by 3.2 kg/m 2. Improvements were most pronounced among participants with high adherence to both diet and exercise components. No adverse events related to the intervention were reported.Conclusions A structured, supervised lifestyle program can effectively reverse early NAFLD and improve metabolic parameters in primary care settings. These findings highlight the potential of non-pharmacological interventions in preventing disease progression and reducing long-term hepatic complications. Wider implementation of such programs could have substantial public health benefits, particularly in populations with rising obesity and metabolic syndrome prevalence.",
