  "abstract": "Background Colorectal adenocarcinoma and inflammatory bowel diseases have distinct etiologies. However, a unifying factor for both groups of disorders is the presence of an inflammatory response in the gut. Chronic inflammation induces significant alterations in the structure of the microbiota and, consequently, leads to changes in the secretory activity of bacteria. In the present study, we compared the secretory activity of bacteria in colorectal adenocarcinoma and in specific forms of inflammatory bowel disease (Crohn’s disease and ulcerative colitis) in order to evaluate the potential applicability of this approach for distinguishing between these groups of ­pathologies.Methods The study was conducted for groups of adults (age 45 to 70) patients with a verified diagnosis of Crohn’s disease and ulcerative colitis and a group of adults (age 45 to 80) patients with a verified diagnosis of colorectal adenocarcinoma. The total number of stool samples was 85. Stool samples were analyzed using a Shimadzu QP2010 Ultra GC/MS with a Shimadzu HS-20 headspace extractor. Results The secretory activity of gut bacteria in inflammatory bowel diseases was markedly elevated. Among the principal alterations observed were significant changes in the levels of short-chain fatty acids, aldehydes, and phenolic derivatives. In particular, patients with ulcerative colitis exhibited a pronounced increase in valeric acid levels, whereas in Crohn’s disease, a reduction in medium-chain fatty acids was detected. In contrast, the assessment of fatty acid profiles in patients with colorectal adenocarcinoma did not reveal substantial alterations in the spectrum of volatile metabolites. Thus, significant metabolic changes were identified in the inflammatory bowel disease group—specifically in Crohn’s disease and ulcerative colitis—while no comparable changes were observed in colorectal adenocarcinoma.Conclusions These findings suggest that gas chromatography-based analysis of bacterial metabolites may serve as a useful approach for verifying ongoing chronic inflammatory responses and could be applied for the assessment of disease status in inflammatory bowel diseases.
