  "abstract": "Background Inflammatory bowel disease (IBD) significantly impacts patients’ quality of life, including sexual function. While erectile dysfunction (ED) is recognized in IBD patients, the association remains poorly understood due to the heterogeneous research findings and the multifactorial etiology. The aim of this systematic review and meta-analysis is to evaluate the pooled prevalence of erectile dysfunction in IBD patients and assess potential risk factors.Method A comprehensive search of PubMed, Scopus, Embase, Web of Science, and Cochrane Library was conducted. Observational studies reporting ED prevalence in male IBD patients were included. We used R software (version 4.3.0) to conduct the meta-analysis. Random-effects model was utilized due to anticipated clinical and methodological diversity across studies. Subgroup analyses were performed based on ED severity ­categories. The risk of bias was assessed using the Newcastle-Ottawa Scale.Results Twenty-one studies, including 29,892 male patients, were analyzed. Study designs included 11 cross-sectional studies, five prospective studies, four retrospective studies, and one letter to the editor. The overall pooled prevalence of ED in IBD patients was 29.6% (95% CI [0.170; 0.464]). Subgroup analysis revealed mild-to-moderate ED prevalence of 18.7%, mild ED 17.0%, moderate ED 6.1%, and severe ED 2.0%. For ulcerative colitis specifically, ED prevalence was 18.4%. Depression emerged as a strong predictor of ED (OR 5.5-13.05), along with active perianal disease, older age, diabetes, and higher disease activity. Significant heterogeneity was observed across studies. With regards to the quality assessment, 13 were rated as good quality, four as fair quality, and four as poor quality.Conclusion Erectile dysfunction is highly prevalent among IBD patients, with depression, disease activity, and surgical interventions identified as key risk factors. Prospective longitudinal studies should establish temporal relationships and evaluate how ED prevalence changes with disease progression. Studies on Crohn’s disease and ulcerative colitis populations separately can provide more precise estimates. Real-world evidence studies incorporating electronic health records and patient registries could help identify predictors of ED in IBD patients and treatment outcomes. Enhanced awareness and targeted interventions are needed to improve the sexual health and quality of life of this population.",
