  "abstract": "Introduction Coeliac disease (CD) is a chronic immune-mediated enteropathy frequently associated with autoimmune endocrinopathies, including thyroid disease and type 1 diabetes. Identifying these comorbidities early is essential to optimize patient management and prevent complications.Methods We conducted a retrospective study of adult patients with CD followed over 30 years (1995–2025) in a tertiary gastroenterology department. Clinical, biological, and histological data were collected. Multinomial logistic regression assessed associations between endocrinopathies and patient characteristics. Statistical significance was set at p < 0.05 (JAMOVI software).Results Autoimmune endocrinopathies were identified in 41 patients (11.9%). Median age was 40 years [27–50]; 61% were female. Endocrinopathies were diagnosed concomitantly with CD in 54% (n=21), prior in 29% (n=12), and following CD in 20% (n=8). Autoimmune thyroid disease (AITD) was most frequent (73.2%, n=30), including 18 euthyroid, 9 hypothyroid, and 3 hyperthyroid patients. Type 1 diabetes occurred in 26.8% (n=11), and one patient had autoimmune hypophysitis. Four patients (9.8%) had ≥2 concurrent endocrinopathies. All patients followed a gluten-free diet.In univariate analysis, younger age, female sex, Marsh III histology, and presence of anti–tissue transglutaminase (tTG) antibodies predicted AITD; multivariate analysis confirmed younger age and persistent anti-tTG positivity as independent predictors (p < 0.01). Risk factors for type 1 diabetes included younger age, female sex, associated autoimmune disease, and non-adherence to a gluten-free diet; multivariate analysis identified persistent anti-tTG antibodies as the sole independent predictor (p < 0.05), reflecting ongoing autoimmune activity.Conclusions Autoimmune endocrinopathies affect ~12% of adult CD patients, predominantly thyroid disease. Routine endocrine screening, especially in young patients or those with persistent serological activity, is essential to enable timely diagnosis and improve clinical outcomes",
