  "abstract": "Introduction Metabolic dysfunction-associates steatotic liver disease (MASLD) is the leading aetiology of liver disease in developed nations and imposes a significant health burden. Therefore, evaluating the temporal trends in the incidence and burden of MASLD is of paramount significance as it enables risk stratification and implementation of preventative measures.Methods Data including the age-standardised incidence rate (ASIR) and the age-standardised Disability-Adjusted Life Year (DALY) rate (ASDR) of MASLD in the United Kingdom (UK) for the period 1990-2023 were retrieved from the Global Burden of Disease database. Joinpoint regression analysis was performed to evaluate the temporal trends in the ASIR and ASDR by calculating the Annual Percent Change (APC) and the Average Annual Percent Change (AAPC). Results were also stratified into the four nations of the UK; England, Wales, Scotland, and Northern Ireland in addition to the different regions within England to evaluate for any national and regional differences, respectively.Results Over the period 1990-2023, an estimated total of 8,050,340 MASLD cases with a male predominance of 61.2% were reported across the UK. England attributed to the vast majority cases, 83.6%, followed by Scotland which attributed to 8.9% of cases. A statistically significant incline in MASLD ASIR was observed across the UK with an AAPC of 0.73 (95%CI 0.72 to 0.74, p<0.001). Stratification into the four nations revealed a statistically significant increase in the ASIR in all nations with the highest incline noted in Scotland with an AAPC of 0.90 (95%CI 0.87 to 0.94, p<0.001), followed by Northern Ireland (AAPC 0.72, 95%CI 0.71 to 0.73, p<0.001), England (AAPC 0.72, 95%CI 0.71 to 0.73, p<0.001), and Wales (AAPC 0.68, 95%CI 0.66 to 0.68, p<0.001). Across the regions in England, a statistically significant increase in the ASIR was observed across the regions with the highest incline observed in North East England (AAPC 0.82, 95%CI 0.77 to 0.87, p<0.001). The lowest decline was noted in West Midlands (AAPC 0.64, 95%CI 0.72 to 0.84, p<0.001).In terms of disease burden, a statistically significant increase in MASLD ASDR was observed across the UK with an AAPC of 3.47 (95%CI 3.39 to 3.58, p<0.001). Additionally, a statistically significant increase in the ASDR was observed across the four nations with the highest increase noted in Northern Ireland (AAPC 4.64) followed by Wales (AAPC 4.12), England (AAPC 3.58), and Scotland (AAPC 2.93).Conclusions The UK witnessed a significant increase in MASLD incidence and disease-related burden across all of its nations and regions over the span of 33 years. These findings warrant prompt recognition to tackle the associated burden.",
