  "abstract": "Background Body mass index (BMI) has been associated with prognosis in several acute and chronic illnesses. However, its role in risk stratification for upper gastrointestinal bleeding (UGIB) remains unclear. This study evaluated whether BMI category alone could predict inpatient mortality in UGIB patients.Methods We conducted a retrospective cohort study of 1,003 patients who underwent inpatient oesophagogastroduodenoscopy (OGD) for UGIB at a tertiary centre between January 2025 and August 2023. BMI’s were categorised as: healthy weight, underweight, overweight, obese and severely obese. Univariate logistic regression was performed against 30-day inpatient mortality as the primary outcome, with further analysis adjusting for UGIB aetiology (variceal vs non-variceal).Results Distributions for the various BMI classes were as follows: 24.3% healthy weight, 4.2% underweight, 27.1% overweight, 16.0% obese, 3.0% severely obese, and 25.4% had no BMI recorded.Underweight patients had significantly lower odds of inpatient mortality (aOR 0.531 CI: 0.295–0.956, p: 0.025), as did those classified as obese (aOR 0.472 CI: 0.262–0.849, p: 0.012). Other BMI categories were not statistically significant predictors. However, the overall model was not statistically significant (p: 0.142), suggesting BMI alone offers limited predictive value, and this was the case regardless of bleeding aetiology.Conclusion Underweight and obese patients had reduced risk of inpatient mortality. Whilst the physiological science remains unclear, these findings support further exploration of BMI as an independent prognostic factor in UGIB, likely with a larger cohort size being required.",
