  "abstract": "Background Accurate preoperative evaluation of tumour size and anatomical location is essential for the optimal management of gastrointestinal stromal tumours (GISTs). Endoscopy and computed tomography (CT) are commonly employed for this purpose; however, their relative accuracy remains unclear.Aim To compare the accuracy of endoscopy and CT scanning in estimating tumour size and anatomical location in gastric GISTs, using final pathological findings as the reference standard.Methods A retrospective analysis was conducted of 42 patients who underwent surgical resection for gastric GIST between March 2018 and September 2025. Tumour size measurements obtained from endoscopy and CT scans were compared with pathological tumour size using mean absolute error (MAE). Agreement in anatomical localisation between endoscopy and CT was assessed in cases where both modalities reported tumour location.Results Endoscopic tumour size measurements were available for 26 patients, while CT scan measurements were recorded for 39 patients. CT scans demonstrated greater accuracy, with a lower MAE compared with endoscopy (14.45 mm vs 19.96 mm). Tumour location was documented by both modalities in 24 patients, with exact anatomical concordance observed in 62.5% of cases. CT scans showed superior completeness of reporting and more accurate localisation, particularly for larger, proximal, and extraluminal tumours.Conclusion CT scanning outperforms endoscopy in both accuracy and completeness for the preoperative assessment of gastric GIST size and location. CT should be considered the preferred imaging modality for surgical planning, especially in cases involving larger or anatomically complex tumours.",
