  "abstract": "Introduction Concerns persist regarding declining academic medicine in the UK. A previous study (Hopper et al., 2009) documented declining abstract-to-publication rates at British Society of Gastroenterology (BSG) annual meetings from 57.6% (1994) to 30.7% (2002). Subsequent analysis showed stabilisation at 17.6% by 2005 (Prendergast et al, 2013). We assessed contemporary publication rates to determine whether UK gastroenterology research continues to decline.Methods All abstracts presented at BSG Live meetings 2021-2023 (n=1,184) were systematically reviewed. Publication status was determined by searching PubMed, EMBASE, and Web of Science databases through January 2025. Primary outcome was full publication rate. Secondary outcomes included lag time to publication, journal impact factors, and associations with presentation type. Chi-square tests compared categorical variables, and odds ratios with 95% confidence intervals were calculated. Historical comparison was made with Hopper et al (1994-2002) and Prendergast et al (1995 & 2005) studies.Results Overall publication rate was 21.6% (256/1,184). Annual rates were: 2021- 26.4% (102/386); 2022- 19.4% (73/376); 2023-19.2% (81/422) (χ 2=7.80, p=0.020). Rates in 2022-2023 (mean 19.3%) were comparable to 2005 rates (17.4%), demonstrating continued stabilisation rather than further decline. Oral presentations achieved higher publication rates than posters (34.4% vs 19.3%, OR 2.20, 95%CI 1.45-3.35, p<0.001). Mean lag time to publication was 15.5 months (SD 8.9), with 85.7% published within 24 months. Published abstracts appeared in high-quality journals (mean impact factor 10.2, SD 8.3), substantially higher than historical rates (mean 3.5-3.9 in 2002). Two-thirds (67.1%) were published in journals with impact factor >4. Top performing sections were IBD (36.8%), endoscopy (32.0%) and gastroduodenal disease (30.8%).Conclusions Publication rates from BSG annual meetings have stabilised at 19-26% over two decades, with no evidence of continued decline since 2005. For trainees, presenting at BSG Live represents an important milestone, with one in five abstracts achieving full publication. Trainees should view submission to BSG Live as a critical step toward full publication rather than an endpoint, with continued mentorship support needed to overcome barriers and maximise conversion to peer-reviewed publications.References Hopper AD, Atkinson RJ, Razak A, Rahim A, Perera A, Jones G, Sanders DS.
