  "abstract": "Introduction Pancreatic steatosis is increasingly recognized as a precursor to metabolic dysfunction and pancreatic ductal adenocarcinoma (PDAC). Non-invasive biomarkers for its detection are poorly defined, particularly in PDAC patients. This study aimed to evaluate serum lipids, apolipoproteins, and nutritional markers as potential indicators of pancreatic steatosis.Methods We conducted a retrospective pilot case-control study including 80 participants. Cases (n=40) were PDAC patients with pancreatic steatosis (pancreas-to-spleen attenuation ratio ≤0.71 on non-contrast CT). Controls (n=40) were age- and sex-matched individuals without PDAC or pancreatic steatosis and without a history of fatty liver, pancreatitis, cholelithiasis, or cancer. Clinical data—including BMI, type 2 diabetes mellitus (T2DM), hypertension, dyslipidemia, alcohol intake, and smoking—were extracted from medical records. Serum biomarkers reflecting lipid metabolism and nutritional status were categorized using established clinical thresholds. Multivariable logistic regression identified independent predictors of pancreatic steatosis, adjusting for relevant covariates. Diagnostic performance was assessed using the area under the receiver operating characteristic curve (AUC).Results In multivariate logistic regression, both low HDL and low prealbumin levels were independently associated with pancreatic steatosis (HDL: odds ratio (OR) 10.71; 95% CI 1.96–58.67; p = 0.006; prealbumin: OR 13.05; 95% CI 2.99–56.99; p = 0.001). T2DM was also significantly associated (OR 8.85; 95% CI 1.35–58.01; p = 0.023). ROC curve analysis demonstrated fair-to-good discriminatory power for both biomarkers, with AUCs of 0.763 for HDL and 0.750 for prealbumin. The difference in diagnostic performance between the two was not statistically significant (p = 0.836). At optimal thresholds, HDL and prealbumin achieved specificities of 85% and 90%, respectively.Conclusion Serum HDL and prealbumin are robust, independent indicators of pancreatic steatosis in PDAC patients.
