  "abstract": "Gastroparesis (GP) is a recognized complication after thoracic organ transplantation that impairs nutrition, medication absorption, and graft outcomes. This study aimed to quantify the risk of GP following lung (LT), heart (HT), or combined heart-lung transplantation (HLT).Methods A systematic search of PubMed, Embase, Scopus, and Web of Science from inception to January 2025 identified studies reporting GP after LT, HT, or HLT in adult cohorts with objective diagnostic testing. Random-effects meta-analysis was used to pool prevalence and incidence, with subgroup analyses by transplant type and meta-regression for age and sex.Results Twenty-four studies, including 2,933 patients, met the inclusion criteria. The pooled prevalence of GP was 30.5% (95% CI: 19.92–43.65%). Subgroup analysis revealed higher prevalence of GP in combined HLT recipients (40.22%, 95% CI: 18.05–67.27%) compared to LT only (26.09%, 95% CI: 15.44–43.65%), though this difference was not statistically significant (p=0.3296). The overall incidence was 26.11% (95% CI: 16.18–39.27%,), with HLT recipients showing higher incidence (37.18%, 95% CI: 23.21–53.69%) than LT only (19%, 95% CI: 8.98–35.81%).Conclusion Gastroparesis affects nearly one-third of thoracic transplant recipients, with a numerically higher burden after HLT transplantation.",
