  "abstract": "Introduction Transgender and gender non-conforming (TGNC) individuals face delays in diagnosis, poorer health outcomes, have high rates of comorbidity and are under-represented in research. 1 Inflammatory bowel disease (IBD) can negatively impact quality of life (QoL) and both diagnostic delays and psychological comorbidity are associated with worse outcomes. There is minimal data examining TGNC individuals with IBD (TGNC-IBD), particularly regarding time to diagnosis, disease outcomes, and QoL and we aimed to study this population in the UK for the first time.Methods Using linked biobank data and a questionnaire, co-designed with TGNC-IBD patients, data were collected from individuals in the UK IBD BioResource who identify as TGNC on demographics, disease features and QoL. Descriptive statistics summarised disease characteristics, and comparisons against an age- and sex-matched cisgender cohort were performed using multivariable regression models.Results 39 TGNC-IBD individuals provided data ( table 1), 2 were excluded from matched analyses as sex-at-birth was missing. TGNC-IBD individuals had a numerically longer time to diagnosis (24.9 vs 18.7 months, p=0.317) and higher rates of IBD related-hospitalisation (64.7% vs 51.4%, p=0.205) and surgery (29.4% vs 16.2%, p=0.066) than cisgender controls. Psychiatric comorbidity was significantly higher in TGNC-IBD (78.4% vs 56.8%; aOR 4.03, 95% CI 1.12–14.46, p=0.033). QoL impairment was substantial across domains, with only small, non-significant differences compared with controls.Conclusion This study provides the first national-level insights into the TGNC-IBD population in the UK and identifies signals of potential disparity in care and healthcare utilisation compared to cisgender controls as well as high rates of psychiatric comorbidity and considerable impact of IBD on QoL.Reference Colwill M, et al.
