  "abstract": "Background Pancreatic cystic lesions and cancers are increasingly identified through cross-sectional imaging, currently require structured pathways and protocols to identify patients at risk of malignancy whilst keeping in mind to avoid unnecessary interventions. This audit evaluates outcomes from a three-month period managed within a dedicated pancreatic cyst pathway at PAH, Harlow.Methods Thirty patients were identified over the period of March- June 2024, all pancreatic cysts were included and those with prior pancreatic cancer were excluded. Variables like age, gender, cyst size, malignancy risk, and subsequent management were analysed to assess pathway performance and appropriateness of follow-up or discharge decisions.Results The cohort had female predominance (women 22, 8men), mean age was 75 with a range of 48yrs- 94years. Cysts sizes varied widely from tiny lesions to a maximum of 60mm. Out of 30 patients, 3 patients turned out to have pancreatic cancer(10%). Out of these three cancer patients, one underwent distal pancreatectomy, one was medically unfit for surgery, and one declined operative management.Regarding pathway outcomes, 40% of patients were discharged from surveillance (12 of 30 patients), 30% of patients ( Nine of thirty) were scheduled for annual MRCP, while one required 6-monthly MRCP. Four patients were referred for surgical triage and follow-up, one required repeat EUS and biopsy, one was placed on 3-monthly surveillance, one attended BBN clinic, one was monitored with serial blood monitoring.Conclusion This three month audit review demonstrates that pancreatic cyst pathway at PAH, Harlow effectively stratified patients by risk with appropriate escalation for suspected malignancy and safe discharge for low-risk lesions. The detection of cancer in 10% highlights the importance of structured surveillance. Another important takeaway from audit highlighted that radiologists should adhere to standard pancreatic cyst reporting format to ensure uniformity.",
