  "abstract": "Introduction The incidence of small pancreatic cysts (<10 mm) has increased with widespread use of cross-sectional imaging for varied indications. Studies have shown that cysts <10 mm rarely develop worrisome features and may not require frequent imaging, highlighting the need to optimise surveillance strategies.Methods We carried out a single-centre retrospective cohort study at University Hospital of North Tees and included all adult patients with newly diagnosed pancreatic cysts identified on cross-sectional imaging between 1st January 2013 and 31st December 2022. All patients were followed up until 31st December 2025.Patients with prior pancreatic cancer or pancreatitis were excluded. Continuous variables were summarised using mean or median values, as appropriate, and compared using Kruskal–Wallis or ANOVA tests. Categorical variables were compared using Chi-square or Fisher’s exact test, as appropriate. Costs were estimated using NHS reference costs (£120 per scan, £150 per new clinic appointment).Results 302 patients were included in the study, 97 (32%) had cysts <10 mm. None of the cysts <10mm developed any high-risk features or required surgery. Table 1 shows the comparative data between different cyst sizes.Conclusions In our study small pancreatic cysts (<10 mm) rarely progressed (above 1 to 2mm per year, N=4) or required any intervention during the study period. Ongoing surveillance in this group has significant clinical resource and financial implications. These findings support consideration of reduced-intensity, risk-stratified surveillance in this select cohort of patients (N=97). Limitations include the retrospective nature of the study with modest median radiological and clinical follow-up.
