  "abstract": "Introduction Bile acid diarrhoea (BAD) is a frequent cause of refractory chronic diarrhoea, often misdiagnosed as irritable bowel syndrome with predominant diarrhoea or functional diarrhoea. Few studies have analysed factors that might be associated with positive SeHCAT in patients with chronic diarrhoea. Our aim was to compare the clinical features of patients with SeHCAT-confirmed BAD, to those without BAD.Methods This is a retrospective, single-centre study of consecutive patients who underwent a SeHCAT test for suspected BAD (January 2016-January 2021). Patients were categorised as BAD and non-BAD based on SeHCAT results (cutoff: 15% retention). Demographic data, symptoms, previous investigations, and current medications were collected from clinical records before the test and compared between the two groups. Data are presented as mean [standard deviation (SD)] or frequency (%). Chi-square or t-tests were used when appropriate.Results 377 patients (64.7% female, age 49.6±16.3) were included, of whom 223 (59.2%) were diagnosed with BAD. BAD had a significantly higher weight, reported having more frequently type 2 diabetes, Crohn’s disease, previous cholecystectomy and ileocolonic resection but no autoimmune thyroiditis, less abdominal radiotherapy and tendency to have less microscopic colitis as compared to non-BAD patients ( table 1). BAD also reported more frequent use of diuretics, inhalers, diabetes therapies and opioids as compared to non-BAD patients (table 1). No differences were found in frequency of gastrointestinal symptoms (table 1).Conclusions This study suggests that despite presenting with similar symptoms, BAD and non-BAD patients seem to differ in terms of comorbidities and concomitant therapies.Abstract P404 Table 1",
