  "abstract": "Background Cancer is associated with poorer outcomes across a range of acute presentations. This study assessed the impact of cancer, metastatic and non-metastatic, on inpatient mortality in patients presenting with upper gastrointestinal bleeding (UGIB).Methods Retrospective cohort analysis of 1,003 patients suspected of UGIB and received inpatient oesophagogastroduodenoscopy (OGD) between January 2025 and August 2023 at a tertiary hospital. Patients were categorised as: no cancer diagnosis, non-metastatic cancer (inc. leukaemia or lymphoma) diagnosis, and metastatic cancer diagnosis. Multivariate logistic regression evaluated whether cancer status independently predicted 30-day inpatient mortality, adjusting for Eastern Cooperative Oncology Group (ECOG) score, Glasgow-Blatchford (GBS), and renal function.Results The cancer status distribution was: 80.1% for no cancer diagnosis, 11.1% for non-metastatic cancer diagnosis, and 8.9% for patients diagnosed with metastatic cancer. As expected, crude rates showed the highest mortality odds in metastatic cancer patients, followed by non-metastatic cancer. Analysis adjusted for GBS and ECOG showed metastatic cancer linked significantly with a higher risk of inpatient mortality (aOR: 2.467, CI: 1.338–4.545, p: 0.004). However, non-metastatic cancer was not significantly associated with mortality in the adjusted model.Conclusion Our analysis suggested metastatic cancer to be a strong, predictive variable for 30-day inpatient mortality, demonstrating the necessity for early recognition and tailored medical management strategies in this high-risk subgroup. In contrast, non-metastatic cancer does not confer significant mortality risk after adjustment.",
