  "abstract": "A 22-year-old patient was admitted to our surgical department with a medical history of abdominal pain and anaemia. His medical history began at age 14, when he started experiencing occasional abdominal pain and vomiting, which were managed with non-steroidal anti-inflammatory drugs. By age 16, he presented with severe anaemia (haemoglobin 8.2 g/L), prompting diagnostic investigations and transient iron therapy. An upper gastrointestinal (GI) endoscopy revealed chronic gastritis, with negative coeliac tests. Persistent symptoms led to magnetic resonance enterography (MRE) in 2022, which showed a partial rotation of the ileal loops. Other imaging and endoscopic studies yielded varied findings but were suspicious of mild inflammatory bowel disease, which was not further confirmed or treated. In 2023, a CT scan confirmed mesenteric vessel malrotation with a clear visualisation of the superior mesenteric artery and vein ( online supplemental video). Subsequently, he underwent an enteroscopy, which provided normal findings extending 120 cm beyond the Treitz ligament. After conducting a patency test, the findings from the capsule endoscopy and a more recent MRE in 2024 (figure 1) indicated ongoing GI issues that necessitate a surgical evaluation.",
