  "abstract": "Background People living with dementia (PLWD) frequently experience health inequalities and often have additional long-term conditions. While ambulance services play a critical role in supporting this population, there is limited evidence exploring the experiences of PLWD who use Patient Transport Services (PTS). This study aims to address this gap by exploring the lived experiences of PLWD when accessing PTS.Methods An ethnographic approach was employed to capture the social realities and lived experiences of PLWD using PTS. This methodology, which is rarely used within the ambulance sector, enabled the use of multiple data collection methods, including direct observation during transport and while waiting, informal conversations, and semi-structured interviews. This approach facilitated participation from individuals who may find traditional research methods challenging and ensured that their voices were represented.Results 50 hours of direct observation and eight semi-structured interviews have been completed. Preliminary analysis has identified key themes including waiting times, opportunities for reminiscence, access to food and drink, availability of information to staff, and the importance of escorts. Findings suggest that PLWD often perceive PTS not solely as a transport function, but also as a meaningful opportunity for social interaction and reminiscence.Conclusion This study highlights opportunities to significantly improve PTS for PLWD, including enhanced staff training, improved booking processes, and consideration of vehicle and waiting area design. The findings aim to inform best practice within ambulance services and acute trusts, contributing to improved patient experience and outcomes. The study also demonstrates the value of ethnography within the ambulance sector as a means of amplifying the voices of under-represented populations.",
