  "abstract": "Background Each year almost five million people died because of the trauma globally. It has been demonstrated that organized trauma systems improve the quality of care provided to patients with serious injuries. After implementation Trauma system in UK, 200% increase in trauma patient seen, secondary transfer decrease from 20%- 16%, increased functional of good recovery outcome 56% to 62%. In last forty years in the kingdom of Saudi Arabia, trauma is the leading cause of disability and mortality.Methods This is a retrospective analysis of two separate sets of prospectively collected from level 1 major trauma center King Fahad Hospital in Alahsa city. Include: any trauma patient has Injury Severity Score 15 and more, and all trauma patients at any age who were hospitalized in King Fahad Hospital in Alahsa city for more than 24 hours. Exclude: any patient who died at arrival Data were collected over two periods: from January 2017 to December 2018 and from January 2022 to December 2023. Statistical analyses will performed using the IBM-SPSS.Results Total 631 patients included in the study in both periods. The sample was divided into two groups: 368 patients (58.3%) were treated in the years 2017-2018, and 263 patients (41.7%) were treated in the years 2022-2023. The majority of the patients were male (87.3%) and of Saudi nationality (73.2%). Motor vehicle accidents (MVAs) were the most common mechanism of injury, accounting for 90.6% of cases. The mortality rate decrease from 61.9% to 38.1% (p =0.011), and length of stay from 13.7 to 11.2 days.Conclusions Significant decrease was observed in the mortality rate from 61.9% in the pre-implementation period to 38.1% in the post-implementation period, and length of stay (LOS) decrease from 13.7 to 11.2. Mortality rate shown decrease in pediatric mortality rate by 50%.",
