  "abstract": "Introduction It is estimated 1,600 patients a year in Ireland suffer major trauma. These patients need to receive the right care in the right place, first time. Our aim was to analyse the differences between major trauma patients who received their entire care in model 3 vs model 4 hospitals.Methods A retrospective review was completed of Major Trauma Audit data looking at patients with an ISS score >15 who received their entire treatment in a model 3 hospital or model 4 hospital between 2017 and 2022. We collected data including patient demographics, injury type, treatments and disposition. We analysed the data to assess differences in patient outcomes between patients treated in model 3 hospitals vs in model 4.Results There were 6641 trauma patients with an ISS >15 treated entirely in a model 3 or model 4 hospital over the 6-year period. There were 2228(33.5%) treated entirely in a model 3 and 4413(66.5%) treated entirely in a model 4. Out of the patients treated entirely in model 3 hospitals 1378(61.8%) of them were male vs 2731(61.9%) in model 4s. There were similar age distributions in both groups with most patients aged between 15–64, 983(44.1%) model 3 and 2110(47.8%) in models 4s. The mean length of stay was 15 days in model 3s and 20 days in model 4s. 388(17.4%) patients died from their injuries in model 3 hospitals compared with 563(12.8%) treated in model 4s. 70(3.1%) of patients were discharged to rehabilitation from model 3 hospitals compared with 416(9.4%) of model 4 patients.Conclusions This study shows a significant proportion of the sickest trauma patients received their entire care in model 3 hospitals. We found these patients to have higher mortality rates and lower chances of being discharged to rehabilitation. The major trauma network needs to rectify this. *presenting author",
