  "abstract": "Introduction This study investigated whether regular departmental in-situ simulation (ISS) in trauma resuscitation improves time to Computerised Tomography (CT) for major trauma patients in the Emergency Department (ED).Methods Over a 10-week period, ten weekly 30-minute ISS sessions were conducted in the ED of St James’s Hospital, Dublin. Each involved Emergency Medicine trauma team members and comprised a pre-briefing, trauma simulation scenario, and structured debrief. Simulations used a Laerdal MegaCode Kelly™ manikin with SimPad Plus and iSimulate ALSi software. Two cohorts were compared: pre-intervention (July–December 2023) and post-intervention (February–June 2024).The primary outcome was time from ED arrival to initiation of CT imaging in actual trauma patients.Results A total of 78 major trauma cases were analysed (40 pre-intervention, 38 post-intervention). Median time to CT decreased from 73 minutes to 41 minutes, representing a 43.8% reduction (p = 0.033) ( figure 1). Mean time to CT reduced from 88 to 61 minutes. The proportion of patients scanned within one hour increased significantly from 43% to 66% (p = 0.040). Although CT imaging within 30 minutes rose from 28% to 45% between groups, this did not reach statistical significance (p = 0.264). Distribution analysis confirmed a significant shift towards earlier imaging post-intervention (Kolmogorov-Smirnov test, p = 0.019).Abstract #304 Figure 1Boxplot showing the distribution of time to CT in pre- and post-intervention groups.
