  "abstract": "Introduction Clinical incident reviews have identified potentially unsafe discharge of Manchester triage category 2 patients, necessitating a review of overall Emergency Department (ED) discharge practice. Additionally, we aimed to identify predictors of return visits within 72 hours of ED discharge.Methods Ethical approval was obtained prior to data collection. This is the first interim analysis of a retrospective cohort study. Data was gathered and analysed with SPSS 30.0.Results 331 patients were identified in the time period, of which, 200 patients, were included in the first analysis. Reasons for exclusion were: age <16 years (n=106), discharge by inpatient service (n=15), missing notes (n=5) and direct specialty review (n=5). The average age was 43.6 years (SD 19.8), while 50.5% of patients were male. Thirty eight percent of patients were brought by ambulance and the average time to be seen was 81 minutes. Patient experience time averaged 6 hours 52 minutes. Most patients (54%) were seen by a Senior House Officer, the most junior medical staff member. In 26% of cases a senior opinion was not sought or documented prior to discharge. A discharge diagnosis was not obvious in 13.5% of cases and 61% of patients did not have red flag advice documented. Only 19 patients (9.5%) represented within 72 hours of which 3 (1.5%) were found to have significant medical issues.Conclusion Senior decision makers need to oversee the discharge of any patient triaged as category 2 at any point in their episode of care. Safety netting needs to be clearly documented at the time of discharge and clearly communicated to patients. Further analysis of the entire patient cohort will help identify any patterns or events that may predict a higher likelihood of unscheduled return. *presenting author",
