  "abstract": "Introduction A descriptive analysis was performed on major trauma patients with head injury (HI) admitted to the Cork University Hospital (CUH) Clinical Decision Unit (CDU). The aim was to assess the burden of major trauma patients with HI admitted to CDU and identify factors correlating with increased length of stay (LOS) or progression to neurosurgical admission.Methods Data regarding major trauma patients with HI admitted to CDU from June 2023 to July 2025 was analysed. The patients were divided into LOS < 48 hours and LOS > 48 hours (range 3–5 days). Data involving demographics, mechanism of injury, toxicology, CT brain (CTB) findings, and neurosurgical consultation/admission were collected for each group by independent researchers. The data from each cohort was then evaluated for indicators of increased LOS.Results The analysis revealed 128 of 394 major trauma patients were admitted to the CDU with HI over two years. Nine of these progressed to neurosurgical admission. Of those discharged from CDU, 73 patients had LOS <48 hours and 46 patients had LOS >48 hours. Both cohorts had a higher incidence of males than females with the same mean age of 45.7 years. On comparing mechanism of injury, road traffic collisions (RTCs) correspond with a longer admission (26.1% >48 hours vs 1.4% <48 hours) and assaults/blows associated with LOS < 48 hrs (27.4% <48 hours vs 10.9% >48 hours). Multi-compartmental injuries on CTB tended to have increased LOS; 32.6% in LOS >48 hours cohort vs 9.5% in LOS <48 hours group. Contrecoup injuries were associated with longer stays (17.4% >48 hours vs 2.7% <48 hours).Conclusion Head injury accounts for 32.5% of major trauma patients admitted to CUH CDU. Despite similar patient demographics in both groups, RTCs and multi-compartmental and contrecoup injuries were associated with increased LOS. *presenting author",
