Harm is experienced within healthcare settings with key areas of risk including medication harm, deterioration, sepsis and hospital-acquired infections. 1 Children, young people and their families (CYPF) are often best placed to work alongside healthcare professionals to reduce safety risks for themselves and the system as a whole.",
