  "abstract": "Nurses play a pivotal role in palliative care through symptoms assessment and timely intervention. This study aimed to explore the experiences of registered nurses in administering breakthrough medications to terminally ill patients. Semi-structured focus group interviews were conducted with 20 registered nurses from a tertiary hospital and an inpatient hospice in Singapore from 2023 to 2024. Data were analyzed through comparative analysis, coding and theoretical sampling iteratively until saturation. Three themes emerged: Concordance, Empowerment, and Helplessness. Concordance, shared understanding and communication within the nurse and among nurses, family, and medical team, determined if the experience of breakthrough administration was supportive or distressing. When concordance was achieved, nurses felt empowered, showing confidence, autonomy, and proactive care. When lacking, nurses experienced helplessness marked by moral distress and passivity. These relational dynamics, shaped by contextual and interpersonal factors, influenced nurses’ subsequent response to symptomatic patients.",
