  "abstract": "Background Post-COVID, we wanted to change our previous ‘Day Hospice’ model and develop a Living Well programme supporting patients earlier in their trajectory, and offering an individualised programme of support.Aim To develop patient-focused services, giving our patients the tools and support needed to live as well as possible, for as long as possible, and to offer practical and emotional support to carers.Method Patients are assessed on referral using the Integrated Palliative care Outcome Scale (IPOS) to support a holistic assessment. Based on their needs, patients are offered a choice of groups and supported by a range of healthcare professionals. Our Living Well service runs a variety of face-to-face and online groups. We have two 8-week programmes; one is called ‘Living Well with Illness’ and the other is the ‘Creative’ programme. Other groups include Art, Exercise, Breathlessness management, Adapted Seated Tai Chi, Social and Therapeutic Horticulture and online guided relaxations. We also run drop-in sessions, and we hold a range of one-off ‘event’ days. Support for carers includes coffee drop-in sessions, pamper days, and a 6-week online programme, covering future planning, coping strategies and what to expect when someone is dying.The IPOS questionnaire is used to monitor the ongoing and changing needs of our patients. The Living Well programme has been designed and modified based on patient feedback and their needs, and their involvement is encouraged in planning.Results We have had great feedback from patients and carers about the service as well as IPOS analysis showing improved outcomes in physical and psychological wellbeing.Conclusion The Living Well programme continues to evolve with input from our patients and carers. It is a mix of online and face-to-face, planned 8-week programmes, one-offs and drop-in sessions which encompass all their holistic needs to ensure they continue to live full active lives for as long as possible.",
