  "abstract": "Background The EDiPPPP study (2023) concluded that whilst a majority of hospice inpatients are eligible to donate their eyes, and there is a desire from both service users and providers to discuss donation, discussions are had with fewer than 4% of eligible patients (Long-Sutehall, Bracher, Mollart, et al. Health Soc Care Deliv Res. 2023 Nov;11(20):1-159). From audit of the local 16 bed hospice inpatient unit, eye donation discussions were only patient-initiated and there had been two successful donations over five years.Aims Up-skill and support staff to achieve a sustainable culture shift toward practitioner-initiated discussions on eye donation.Establish and demonstrate the hospice as a key facilitator of eye donation referrals.Methods Implementation of STEPS (Support Toolkit for Eye donation in Palliative care Settings: Long-Sutehall, Bracher, Mollart, et al. Health Soc Care Deliv Res. 2023 Nov;11(20):1-159) and other bespoke education.Creating a visible presence for eye donation information in the hospice.Integration of eye donation eligibility checking and discussions into everyday practice including multi-disciplinary team meetings, handover, admission clerking.Electronic flowchart for eligibility, discussion, referrals from the inpatient unit and post-discharge.Electronic Patient Record development to propagate and underpin change – electronic eligibility checklist, centralised record of eligibility, discussions and patient consent with associated read codes, automated inpatient and external alerts for consenting patients.Sessions to publicise local patient and relatives’ donation stories.Results Over a three-month period:Eye donation eligibility was determined for 100% of hospice inpatients (n=76).Eye donation discussions were had with 50% of eligible patients (n=48).11 referrals for eye donation and six successful donations.Conclusions Meaningful culture shift in both hospice staff and patients can be achieved through a systematic and comprehensive approach to change, as demonstrated by this hospice’s newly established culture of facilitating patient eye donation. Keys to success include bespoke education and training, system change to embed into everyday practice, supporting change through electronic systems and promoting and celebrating success.",
