  "abstract": "Background The number of children and young adults living with a life-limiting condition has more than doubled since 2000, with one of the highest prevalences in North-West England (Fraser, Gibson-Smith, Jarvis, et al. Palliat Med. 2021;35(9):1641-51). Equipped with this knowledge, and a shared concern regarding the provision of hospice care for young adults due to there being no formal transition pathway in existence within the local area, a collaboration began between our children’s and adult hospice.Aims To develop a transition pathway for young adults. To create an adult hospice space tailored to young adults. To share learning widely.Method Scoping exercises were undertaken regarding transition services in 13 adult and nine children’s hospices. Service user interviews took place with ten young adults and parents to understand their views on a transition pathway and their preferences regarding an inpatient unit space. The information obtained from both scoping exercises was summarised via content analysis using NVivo14 software.Results Hospice scoping identified common themes including multidisciplinary team working to triage, personalised care and introducing young adults and parents to the adult hospice early. Young adults and parent engagement yielded similar findings and informed plans for an adult inpatient room. Popular design features included neutral décor, modern interchangeable artwork, access to a smart television, and ensuite with hoist. These opinions have been used to develop a mood board for a capital project which is due to start in July 2025. The hospice is committed to sharing project learning as widely as possible.Conclusion Through a national scoping exercise, and service user interviews and engagement, we have developed a proposed clinical pathway for young adults transitioning to adult hospice care. We hope that this learning will be valuable to other regional hospices, and indeed to the wider palliative care community.Funder Masonic Charitable Foundation grant managed by Hospice UK.",
