  "abstract": "Background Referrals are received using a pan-London referral form along with supporting information. Referrals are triaged based on information gathered, a RAG rating system and some gut feeling.Aim To explore the challenges and benefits of applying a specialist triage tool within a hospice: a service development project incorporating the Responding to Urgency of Need in Palliative Care (RUN-PC) triage tool into the admissions and referrals process using ADKAR model (Hiatt. ADKAR: a model for change in business, government, and our community. Prosci; 2006) to implement change.Method Using a multi-method approach to data collection, questionnaire, pre and post triage scores and focus group following a four week trial of the triage tool. A purposive sample was chosen, inviting the First Contact team (comprising Band 6 and 7 nurses plus a team leader and Head of Service) to participate in the data collection. Data was analysed comparing scores and using a reflexive thematic analysis approach.Results The analysis of the questionnaires, pre and post training scores and focus group highlighted several benefits and challenges. The benefits were; urgent referrals were clearer, the tool evidenced decision making and staff were positive about the change, e.g. it ‘ Enables clear goal for time of response’ and it provides ‘evidence to back our decision’. The challenges that emerged were; many referrals lack information to triage effectively, concerns about consistency of scoring and increase to workload. In the focus group the staff were keen to continue to use RUN-PC triage tool.Conclusion RUN-PC is a useful tool to aid triage and prioritise patients and was positively received by the participants. An action plan was developed to improve the use of the tool by embedding it into the service. This service development also helps the hospice to align with NHS and NICE end of life care guidelines.
