  "abstract": "Introduction Palliative and end of life care virtual wards provide a safe alternative to inpatient hospital or hospice care for patients who become clinically unstable or at high risk of deterioration but who could, and wish to, be cared for in their usual place of residence. The Palliative Response Team (PRT) was developed, in collaboration with local partners and Rennie Grove Peace Hospice Care, following a review of traditional hospice at home models which identified the ability to respond promptly to crises/unplanned care as the main gap in community palliative care services. The Palliative Response Team was developed as a virtual ward and staffed by an innovative skill mix of band 6 nurses and paramedics, clinically supported by a palliative medicine consultant.In this poster we will present the results of two identical snapshot audits completed nine months apart.Methodology Patients were identified from SystmOne ( electronic patient record) between June and August 2024, and March and April 2025. All referrals were reviewed to understand their eligibility criteria into the service. In addition, all activity during these periods was reviewed by the consultant and other auditors looking at: referral priority, the need for symptom control, advance care planning and admission avoidance and outcomes.Results 84 referrals were reviewed. Interventions, outcomes and admission avoidance will be presented.Limitations Even with two reviewers, many of the referral reasons could have been relevant for >1 criteria, so a judgement was made as to which was the most appropriate.Conclusion Most patients were deemed unstable needing a review and intervention within 4-24 hours. We would conclude that timely Palliative Response Team intervention avoided hospital attendance and reduced pressure on acute services.",
