  "abstract": "Background The Hospice of the Valleys’ In-Reach Service delivers an integrated model of specialist palliative care across 14 care homes, Ysbyty Aneurin Bevan (YAB) community hospital palliative beds, and Ysbyty Tri Chwm (YTC) mental health unit.Aims The service aims to provide consistent, expert palliative care to patients, families, and staff in care homes, community hospitals, and mental health settings. It also aims to reduce avoidable hospital admissions and improve patient outcomes through education, collaboration, and continuity of care.Methods A Clinical Nurse Specialist (CNS) with educator/facilitator responsibilities and an Advanced Nurse Practitioner (ANP), the only palliative care ANP and independent prescriber in this context, leads the service. Key activities included managing referrals, providing inpatient support, delivering training, and enhancing systems for documentation and collaboration. Data was collected on patient referrals, caseload, end-of-life support, training hours, and participant feedback.Results Between April 2024 and February 2025, the service received 109 care home referrals and maintained a monthly caseload averaging 34.4 patients, with 78 supported through end-of-life care. At YAB, 140 new patients were referred: 50 from out-of-area and 90 from Blaenau Gwent. The service delivered 105 hours of training to 279 participants, who reported increased confidence in symptom management. Collaborative care planning and seamless CNS-ANP coverage helped reduce avoidable admissions.Conclusion The In-Reach Service demonstrates how embedding hospice expertise within community and hospital settings can improve care quality, promote dignity, and enhance person-centred outcomes. An ongoing partnership with Aneurin Bevan University Health Board supports the sustainability and expansion of this model. Innovations in documentation, physical environment, and multidisciplinary team working have further enhanced service delivery.",
