  "abstract": "Background Community Hospice supports 535,000 residents across Greenwich and Bexley, yet around 20% of patients are from ethnically diverse backgrounds. A Community Engagement Manager was appointed and interest from many communities quickly grew. With limited resources, we recognised the need for a prioritisation matrix to engage all communities at appropriate levels.Aim To develop our approach to researching equitable, culturally responsive engagement at varying levels of need.Method In January 2023 we appointed a Community Development & Engagement Manager. Using 2021 census data, we determined the three largest heritage groups uniquely concentrated in Greenwich or Bexley and worked closely with them. As relationships spread to other communities we sensed the risk of thinning resources, so we designed an asset-based ‘Community Matrix’. It weighs four factors (population, unmet need, enthusiasm/assets, and strategic fit), with enthusiasm and access gap counting most. Twenty-four groups are scored annually: we work intensively with the highest scoring, others join an ‘opportunity pathway’ of advice, shared events and drop-ins, ensuring fair, culturally sensitive end-of-life care.Results Priority communities received intensive engagement, including six-week culturally tailored workshops delivered to Nepalese and Vietnamese groups; Nigerian pastors attended annual hospice summits and community education days. Two smaller but motivated groups (Lithuanian and South Asian) piloted the Opportunity Pathway, leading to a monthly ‘Chai Shai’ group and engagement with a local Lithuanian factory. This approach has also recruited six new community engagement volunteers, enabling staff to focus on priority communities while sustaining broader connections.Conclusion A concise, evidence-based matrix helps a hospice decide where to start yet remain open to wider partnerships. Publishing data, tools and insights – through the South East London ICS Neighbourhood & Health Inclusion Community of Practice and the Healthier Greenwich Partnership – enables us to direct our time where it matters most.",
