  "abstract": "The aim of the project was to be reactive to ‘The Inbetweeners’ report (National Confidential Enquiry into Patient Outcome and Death; 2023) and listen to the voices of the young people in our hospice, currently under the care of a neighbouring children’s hospice, where no transitional pathway is present into our adult services. We wanted to engage young people and their loved ones, equip our facilities to be comfortable and age appropriate alongside upskilling our staff through education/training; creating a pathway which would establish a transition framework for future collaboration.We approached the project in phases, sending our staff for training days and shadowing visits at the neighbouring children’s hospice. We invited the children’s hospice’s transition coordinator into our hospice and held three family days, gathering patient and family initiated feedback from user involvement for ideas for a young person’s space. This led to the designing and refurbishing of a room bespoke for families.From the three family days held, three families attended the first event, the second event saw two re-occurring and two new families, and the third event, two existing families and a new family. This totalled 50% of engaged family support from the catchment caseload of 12 patients/families held at the neighbouring hospice.The project supported a wider system review into the feasibility of a transition pathway across the region, supporting age appropriate facilitated care within a developmentally appropriate environment. The continuity and connectivity of the working partnerships with the collaborating hospice allowed for future development of family days. Learning and feedback from the events demonstrated that families enjoyed the opportunity to meet like-minded people in a safe environment, learn more about end-of-life care and gain a better ability to access more support for themselves.
