  "abstract": "Background Enabling high quality end of life care for care home residents is essential, preferably enabling the resident to remain in the care home at end of life. Key requirements include recognising dying, advance care planning (ACP), and staff being supported with education and training.Aims of the project Staff to develop knowledge and skills to support residents to remain in the care home at end of life. Registered Nurses to develop competence to assess and manage end of life symptoms (including syringe pumps).Methods 26 x 90-minute focus/support groups were delivered to 246 staff in 23 nursing homes exploring delivering end of life care, then introducing the REDMAP 3 ACP framework, the SHADOW tool (NHS England. Early Identification in Care Homes (SHADOW) [internet]) to help with early identification of the last year of life, and a planning checklist. The Registered Nurses were encouraged to attend additional education at the hospice to increase their confidence in end of life care clinical decision making, and further support clinical skills competency attainment. 14 x 60-minute focus groups were facilitated in 13 residential homes (to 149 staff) introducing the same models, with carers encouraged to attend the hospice’s current education programmes.Results Two primary themes emerged from the focus group discussions:Difficulty in recognising dying.Managing families’ expectations around care needs.Education outcomes Subsequent increase in education attendance at the hospice for both Registered Nurses and non-registrants. Twenty bespoke syringe pump teaching sessions delivered to 150 Registered Nurses.Conclusions The collaborative project was part of a series of interventions to support high quality care in care homes, resulting in a 35% decrease in 111 calls and hospital attendance of care home residents over this period. It enabled engagement with some care homes where there had previously been none, with an 80% increase in staff’s education uptake to increase their skills, competence and knowledge.",
