  "abstract": "Background As an all-age service we knew how many young people were transitioning out of our children’s service, and how many young people were accessing our adult service, and the disparity between these numbers. We were not fully tapping into the potential benefits of being an all-age service, especially since the loss of a dedicated young adult hospice in Essex. Families were telling us that there is limited transition support across healthcare particularly for those with complex health and palliative care needs and learning disabilities.Aims To employ a Youth Worker to holistically support young people (aged 11-25) accessing the hospice with the aims of achieving the following;Individually and within groups, remove barriers to adult services.To ensure Youth Voice is heard and embedded into all aspects of our services.Young people to feel supported to engage with and access adult hospice care both within our service and more widely.Continuing holistic support of the young people after referral to adult services to ensure continued and successful engagement.Methods The job description for the role was written with input from transition, health and youth work professionals. Youth Worker recruited. Time taken to scope the need, establish roles and responsibilities. The existing cohort of young people were engaging with the children’s hospice service so time taken to engage with this group, seek feedback and learn lessons to take across all services.Results A significant culture change from having a ‘children’s service’ and an ‘adult service’, to having an ‘all-age service’ with the right people providing care in the right place at the right time.Conclusion The introduction of new roles within the multidisciplinary team can create new ways of thinking and working for the benefit of young people.This project was funded by a grant from the Masonic Charitable Foundation managed by Hospice UK.",
