  "abstract": "Background At St Andrew’s Hospice we deliver both adult and children’s palliative and end of life care and therefore have a duty to support young people and their families through their transition into adulthood. The number of young people with life-limiting conditions is increasing, with a growing number living into adulthood. In 2001/2002, 921 young adults lived to aged 19 compared to 3,075 in 2017/2018 (Fraser, Gibson-Smith, Jarvis, et al. ‘Make Every Child Count’: Estimating current and future prevalence of children and young people with life-limiting conditions in the United Kingdom. 2020).Aims/purpose The Circle Club aims to support young adults by:Preparing them for adulthood – building confidence, forming friendships.Preparing for moving on – life skills.Settling into adult services – building relationships.Methods Young adults (16+) accessing Andy’s (children’s) services are identified and discussed through regular Transition multidisciplinary team meetings. This identifies the young adult’s needs, and which services may best suit this individual. Wellbeing staff are introduced to the young person and their families/carers. Circle Club trial is offered. Andy’s staff work alongside Wellbeing staff to support the young adult to build relationships within adult services. Once the young adult feels at ease within adult services, they are discharged from Andy’s. Regular three-monthly reviews are held, focusing on person-centered goals and wishes. Transition support continues with other services being offered, i.e. respite in the inpatient unit, if appropriate.Results Circle Club currently cares for five young adults:One young adult has completed the transition process.One young adult is currently being transitioned.Three young adults were referred into the wellbeing service.Conclusion Circle Club has proven to be a supportive, effective group to meet the needs and care for a younger adult population. St Andrew’s Hospice is demonstrating that by Adult and Children’s services working collaboratively a new model of service delivery is possible that meets the needs of a growing population of young adults with a palliative condition.",
