  "abstract": "Background Care overnight in the community was provided by the hospice and the community nursing service. It was recognised there was duplication of care, with limited integration.Integrating the two services provides a single point of access for patients, families and healthcare professionals out of hours.Aim Patients who require an unplanned visit in the community receive continuity, seamless care and no duplicate call outs. The overnight service responds to crisis calls preventing admission to hospital.Method From April 2024 the hospice provided SinglePoint of Access for all out of hours community calls between 20:00 and 08:00, seven days a week. The criteria is palliative end of life care, catheter care and bowel management. If a crisis visit is required, the patient should expect to receive a visit within two hours, following the initial triage call completed. If a patient or healthcare professional requires a community nursing visit the next day a referral will be taken and passed over to the appropriate community service. The community nursing team attend daily hospice multidisciplinary team morning meetings enabling complex patients to be discussed and a mutual plan is agreed which is patient and family centered.Results From April 2024-March 2025 2,570 visits were carried out, 59% of these visits were for catheter care, 14% stat doses, 9% pain management, 6% non-pain symptom management and 4% syringe driver issues. Some of the other reasons for visits included deteriorating conditions, general advice and deceased patients. 85%-92% of patients received a visit within two hours.Conclusion The night integration service has been successful. Initial concerns were around non-palliative patients receiving hospice care. There have been no concerns raised from patients or their family regarding this. The service is now running seamlessly providing continuity and seamless care with no duplication of visits.",
