  "abstract": "Background 43.4% of people in England die in hospital (Office for Health Improvement & Disparities. Palliative and end of life care profile December 2023 update: Statistical commentary. 2024 Sept 17. [internet]), but most prefer to die elsewhere. Hospices are recognised as a gold standard place to die. The scheme allows the transfer of patients nearing the end of life with non-complex needs the opportunity to be given a hospice bed who otherwise may not be prioritised. This is limited by transfer times and bed availability.Aims To improve access to hospice care for hospital inpatients with short prognoses and non-complex needs. To enhance patients’ experiences and alleviate pressure on inpatient hospital beds by collaboratively developing and evaluating a nurse-led hospice admission model.Method Palliative care teams at an acute hospital trust and local hospice jointly developed the model of care and eligibility criteria. Potentially eligible patients are identified and reviewed early in the day by a palliative care specialist nurse and consultant. Regular communication between the hospice and hospital teams regarding referrals and bed availability is enhanced by the appointment of a liaison nurse based at the hospice. To monitor the service, data is routinely collected on patients identified, referred and transferred and on nurse-led bed occupancy. We analysed this data to evaluate process and care outcomes.Results In 2024/5 median bed occupancy was 98% saving 1,898 acute bed days at the hospital. 198 patients were transferred, of these 16 patients were discharged from the hospice for longer term care in the community. Informal feedback from those who had been successfully transferred to the hospice has been overwhelmingly positive.Conclusion This nurse-led model demonstrates how collaboration between acute hospital and hospice services can facilitate timely transfer to a hospice setting for inpatients who may not otherwise have been considered. This model could be used elsewhere to increase uptake of hospice care for those dying without complex needs.",
