  "abstract": "Background In our rural county there are no other services to move our young adults with complex needs into. As a joint adult/children’s setting we considered how we could meet their needs in adults’ setting and used a PDSA (Plan, Do, Study, Act) approach. Our inspiration was a young person – despite meeting the criteria for NHS Continuing Healthcare (CHC) funding, there was simply nowhere his mum could access useful support to meet his needs, or her needs.Aims Using the skills of the children’s and adult teams, to meet the needs of our young people. To explore training and funding requirements.Methods We secured CHC funding for the young person – this covers staffing costs. This is important as we cannot care for this cohort from money fundraised to support children. We utilised the PDSA approach and tried a mix of the children’s team and adult team working together.Results We found the children’s team supported our young people to sleep on an adult ward. While it had novel value, it didn’t engage the adult team in supporting care, spreading skills and creating the right environment. The service is now a true mix of children’s and adult support:Increased the number of young people.Ringfenced ‘sessions’ so the young people can use the children’s hospice space in an age-appropriate way.We engaged mixed staff teams to work together in each other’s environments.We made sure our learning disabilities nurse was at the heart of the pilot.We moved to creating a new service.Conclusions We now have four young people in our ‘Young Adults with Complex Needs’ service receiving regular breaks and care which we staff, monitor, review and audit as a standalone pilot service. We are challenging and developing our staff and ensuring the funding underpins roles.",
