  "abstract": "Background In order to provide ‘a purposeful and planned’ (Blum, Garell, Hodgman, et al. J Adolesc Health. 1993;14(7):570-6) transition, young people with a life-limiting or life-threatening condition, their families and carers need to be able to receive information and support in an appropriate and timely way. The service was challenged in providing this by a geographical area of five counties and a caseload of 180 young people.Aims To provide information and support to as many young people and families as possible at the appropriate time during their transition.Method Three webinars were developed and invitations issued via email to the following age groups:‘An introduction to transition’, age 13.‘Decision-making in transition’, ages 13 to 20.‘Transition, the next steps’, ages 15 to 19.Times of 11.30am and 17.30 offered in order to promote inclusion for young people at college and for parents with daytime commitments. Eight webinars delivered to date.Results Webinars attended by 23 young people, parents, family members and carers. Positive evaluation received with regard to timely information, particularly with regard to decision-making and legal aspects of consent throughout transition and parallel planning. ‘Decision-making in transition’ webinar delivered to 25 professionals.‘I feel empowered to be able to speak to medical professionals knowing I have a right to be included in best interest decisions being made on behalf of my Young Person’.‘I never thought I’d be able to go to university but now I know with the right support I can.’Families also appreciated the chance to connect with others in a similar situation.Professional comments included ‘This has been an absolute eye-opener’.Conclusion Continued delivery, evaluation and development of webinars. Future development in line with evaluations to include recording webinars to share for those unable to attend and further development of webinars for professionals.",
