  "abstract": "Background The Care Quality Commission’s annual report (The safer management of controlled drugs: Annual update 2023) identified a key concern of local intelligence networks in relation to diversion of controlled drugs (CDs) in lower Schedules. They acknowledged it was particularly challenging to identify diversion for organisations using these in larger volumes. In May 2024 the Hospice Medicines Management Group could not be assured the risk of diversion was fully mitigated, and more robust processes were required in relation to the safe storage of these medications on the adult inpatient unit.Aims To improve the oversight and monitoring of Schedule 3, 4 and 5 CDs which are potentially drugs of misuse but are not under the same controls as Schedule 2 controlled drugs.To improve and embed new processes that support safe storage and record keeping of Schedule 3, 4 and 5 medication, ensuring clear evidence of monitoring and oversight of these medications.Method Stocks of Schedule 3, 4 and 5 medications removed from general storage into CD cupboards and boxes signed out one at a time. Initially frequent audits and checks completed by inpatient unit managers to provide assurance to Controlled Drugs Accountable Officer and Medicines Safety Management Group. Stock list review to prevent overstocking. Controlled Drug Policy and Procedure updated. Collaborative working with the pharmacy stock technician to support new processes. Oversight visit by CD Liaison Officer to review and advise on safe storage and processes.Results There are now clear, documented processes that provide assurance to the Medicines Safety Management group that Schedule 3, 4 & 5 medications are appropriately stored and managed on the adult inpatient unit, no further concerns about potentially unaccounted for medications since the measure were introduced.Conclusion Enhanced monitoring is now embedded process and remains in place due to increased risk of diversion for Schedule 3, 4 and 5 medications. Sharing of learning through hospice collaborative CDAO group as other hospices are interested in these arrangements.",
