  "abstract": "Background Palliative care is a fundamental human right, requiring a person-centred approach that prioritises the specific needs and preferences of individuals (World Health Organization. Two years ago, Martlets Hospice Inpatient Unit (IPU) began implementing The Lantern Model which has as one of its core tenets, ‘finding the person in the patient’ (https://www.stchristophers.org.uk/lanternmodel).The implementation of ‘What Matters to You?’ (WMTY) enables palliative care professionals to support patients using a person-centred approach. The WMTY question was introduced into the handover process, multidisciplinary team (MDT) meetings and patient records. This audit evaluates how well WMTY is embedded in the documentation and the impact it has had on clinical staff.Methods A mixed-methods approach was used to assess the implementation and impact of WMTY:Quantitative analysis of 20 randomised health records from August – November 2024, reviewing documentation for evidence of WMTY on admission notes (Medical and Nursing), nursing handover template, record of care, MDT template, and discharge letters (Medical and Nursing). Qualitative feedback from palliative care clinicians across the MDT, exploring personal reflections on use of WMTY via a survey. Sample is representative of 27.4% of clinicians.Results WMTY was present in all domains, assessed across 20 records, confirming integration into patient care. MDT records lacked WMTY in three cases, where patients had died before discussion, highlighting an opportunity to embed WMTY into Mortality and Bereavement Reviews. Discharge records showed inconsistent documentation, suggesting a need for standardisation across nursing and medical templates. Staff reflections confirmed that WMTY enhanced communication and fostered meaningful and compassionate engagement.Conclusion This audit demonstrates that the WMTY question is embedded across documentation processes, reinforcing person-centred care as a core principle. The next steps are to:Strengthen implementation; WMTY should be integrated into healthcare records to ensure continuity in compassionate, individualised care.Spread learning across the wider Southern Hospice Group.",
