  "abstract": "Purpose To share the development of interactive advance care planning (ACP) reports from a statewide ACP information system.Background Queensland has a robust ACP information management system, including an ACP database and the clinician-facing ACP Tracker, used by both private and public health professionals, including the Queensland Ambulance Service. Currently, data sources to monitor and measure ACP activities are fragmented. The Statewide Office of Advance Care Planning (OACP) produces static high-level ACP reports for select document types. Separate detailed ACP Tracker reports and performance indicators are available on different reporting platforms. Clinician feedback highlights the need for a more user-friendly interface to consolidate this disparate information.Main Text In response, OACP has developed interactive Microsoft PowerBI reports that integrate statewide ACP information from multiple sources.Key functions include displaying the following metrics in one location:ACP documents received (>207,000)Document issues preventing upload to the ACP TrackerEnd-of-life preferences (70% of responses do not want life-prolonging treatment or cardiopulmonary resuscitation. 40% of responses prefer to die in a residential aged care facility)ACP Tracker comments entered (40,000/year)ACP Tracker launches (1,000,000/year) and document views (120,000/year)The interactive report will be accessible by all Queensland Health staff via the MyCareMyChoices.com.au website. Investigation of options to share this with external health providers (after additional approvals and privacy assessments) is progressing.The introduction of detailed interactive reports has driven enhanced data quality within the OACP database. Better system integration is supporting reduced data errors.Long-term, OACP aims to link its data with broader Queensland Health datasets to enhance patient care and incorporate ACP key performance indicators in future reports.Lessons learned include using state and national datasets with unique identifiers for patients, documents and a provider directory.Unique Contribution Queensland is the first jurisdiction with statewide interactive ACP reports which can also assist organisations in demonstrating compliance with Queensland and national ACP related healthcare standards.Implications The PowerBI reports support Queensland Health services to easily monitor and measure ACP activity and implement related quality improvement activities.Linking ACP data with hospital admission and discharge information in the future, will enable targeted ACP discussions, ensuring patient-centred care for those in need.",
