  "abstract": "Introduction Our current Embletta polygraphy devices (Natus,USA) are due for replacement. We were offered a free trial of the Brizzy+ (BR+) (NOMICS,BE) that uses novel jaw activity technology with other signals to assess sleep apnoea. The purpose of this study was to evaluate its ease of use, reliability, and obtain patient feedback.Methods 31 patients were recruited. They undertook a one-night sleep study with concurrent BR+ and our current first line diagnostic, WatchPAT (WP) (Itamar Medical,NL). The primary outcome was the apnoea-hypopnoea index (AHI), but oximetric parameters, snoring, and sleeping position were also compared. Data were normally distributed, henceforth denoted as mean (standard error). Relationships were assessed by Pearson’s correlation, with a two-tailed paired t-test applied to assess average differences. Bland-Altman plots were used to evaluate agreement. Patients were asked to complete a feedback questionnaire for BR+ relating to ease of set-up, comfort, and transducer reliability.Results Success rate was 97% for WP and 86% for BR+, yielding a final cohort of 26; 12M:14F, mean age 41 years (range 25-59), and mean BMI 35.6 (2.1) kg/m2.AHI was no different on average and correlated strongly (r=0.897, p<0.0001), although the Bland-Altman plot highlighted large individual differences (figure 1). Near identical outcomes were observed for ODI and time below 90% SpO2. Mean SpO2 was higher by WP [92.4(0.76)] than BR+ [91.9(0.80)] (p=0.048) but this was not clinically significant. WP detected more snoring [27% (5.3)] than BR+ [2.7% (0.9)] (p<0.001). There were differences in supine sleeping [WP 55% (6.2), BR+ 36% (6.2)] (p<0.01) and right-side sleeping [WP 12% (2.7), BR+ 29% (5.2)] (p<0.01).84% of patients found BR+ easy to use. Comfort was overall average (most complaints relating to the nasal cannula) and 25% felt BR+ significantly impacted sleep quality. The chin probe sometimes fell off when patients had a beard.Conclusions BR+ is a simple, user-friendly and mostly reliable device that could easily be implemented into our department. Large individual difference in AHI and other parameters do not necessarily indicate inaccuracy of BR+, as both BR+ and WP are only level 3 diagnostics and have independently demonstrated good comparisons to polysomnography.Abstract P41 Figure 1A Bland-Altman plot comparing agreement between AHI measured by WP and BR+ during a concurrent study in 25 patients with suspected OSA.
