  "abstract": "Introduction After establishment of non-invasive ventilation (NIV), ongoing adherence is crucial for long-term treatment success. We previously reported on the role of remote monitoring in paediatric NIV adherence (Thomas, A. et al. Pediatr. Pulmonol. 2021). In the current study, we report follow-up data on remote monitoring five years after NIV establishment.Methods This study was based on the cohort previously described by Thomas et al., comprising of 57 patients (re-)established on NIV during an inpatient stay. Here, we investigated which patients were still actively using their ventilator five years after establishment (based on remote monitoring), and what their adherence and treatment efficacy was in the last 90 days.Results (see table 1): Adherence in the first 90 days after establishment was good (median 6h40min per night). Five years after establishment, 20 of the 57 patients were still actively using a telemonitoring-compatible device. Of these, 13 used CPAP and 7 were on BiPAP. Median NIV usage was 76/90 days, and median treatment duration per night used was 7h10min. Adherence in the patients with versus without neurodisability was 44% versus 22%. Leak was overall acceptable, and the residual NIV-detected AHI was in the mild range (median 2.15/h). There were no statistically significant differences in these outcome parameters for CPAP versus BiPAP patients.Discussion This study provides real-life data on telemonitoring-based adherence and treatment outcomes 5 years after initial NIV establishment. In our cohort, only 35% of patients were still actively using a telemonitored NIV device. This low number likely reflects a true drop in adherence, but also accounts for changes to NIV devices that are not compatible with telemonitoring, having withdrawn consent for telemonitoring, and/or having transitioned into adult care centers that do not offer telemonitoring.
