  "abstract": "Introduction Sleep disordered breathing (SDB) is a term used to define cessation or partial cessation of airflow that results in oxygen desaturation, or an arousal from sleep (Berry et al., 2017). The ‘BTS statement on Paediatric SDB’ states that questionnaires can be used in children ≥2 years old without comorbidities (Evans et al., 2023). The Paediatric Sleep Questionnaire (PSQ) (Chervin et al., 2007) and the OSA-16 (Franco et al., 2000) have been studied in children with comorbidities and have shown varying results. In this study we sought to investigate the sensitivity and specificity of the sleep disturbance scale for children (SDSC) (Bruni, O et al 1997) as an alternative questionnaire to identify SDB in children referred to a tertiary respiratory/sleep service with significant comorbidities.Methodology This was a retrospective analysis of patients (29 Males (58%), 21 (42%) Females) who attended a children’s sleep laboratory for cardio-respiratory polygraphy (CRSS) between the dates of: 07/01/2025- 15/05/2025. 32/50 patients were ENT surgically naïve (table 1 shows patient demographics). All patients included in the analysis were >2 years old and all had baseline studies self-ventilating in room air. Patients attended the sleep laboratory and the SDSC questionnaire was completed by the accompanying parent/guardian prior to the CRSS.Results There was no correlation between the presence of sleep disordered breathing (classified by an AHI ≥5/h) with total score (-0.18) and the SDB-related questions, respectively (-0.02). Sensitivity and specificity of the SCDC severity thresholds are demonstrated in table 2.Discussion This analysis of the SDSC in this cohort of patients, with significant comorbidities, shows no correlation with CRSS results. There is also a lack of diagnostic specificity.
